YOUR VISIT
Please check in at the reception desk so the clerk is aware that you have arrived for your appointment. Plan to arrive at least 15 minutes prior to your appointment to allow sufficient time to complete any paperwork. We will mail the paperwork in advance of your appointment for your convenience. You may also print copies of the Patient Information Forms, or complete and fax them. Please complete these necessary forms prior to your appointment. Be as accurate and complete as possible when completing these forms. This information assists us in providing the best possible care for your particular condition. It is critical that you bring a copy of your current insurance card, any copay that you may owe, an authorization number if required, and any copies of X-rays or other diagnostic studies, such as MRIs or CAT scans. If this is a work-related or personal injury case, please provide employment or liability information. Parking is available on-site on the North side of our building. For the comfort of all patients, we request that you turn off or set your cell phone/pager to silence. The physicians and staff of Neurosurgery Specialists make every effort to accommodate patient schedules and make visits to our office pleasant. We recognize that your time is important. However, sometimes emergency situations may cause a physician to be delayed, leave the office suddenly or have to cancel their office entirely. Neurosurgery Specialists asks for your understanding in the event your visit is altered by such circumstances. |
